Graduate Nursing Program Application Checklist

YOUR APPLICATION MUST BE COMPLETE TO BE CONSIDERED FOR ADMISSION

In addition to the submitted online application, mail completed materials in one packet to the College of Graduate Studies at the following address:

Materials must include:

  • Non-refundable $45 application fee; please submit at time of initial application.
  • Personal statement describing career goals, future plans for employment, and expectations for graduate study.
  • Current resume or CV; listing current Ohio RN license number.
  • Two College of Nursing recommendation forms sealed in envelopes with referee's signature across the envelope seal (from professional sources, Master in Nursing preferred).

You must request the following reports be sent directly to: College of Graduate Studies Admissions Office

  • Official transcripts from all previous colleges and universities.
  • Official Graduate Record Examination (GRE) score report (taken within five years). (Required for Clinical Nurse Leader- Graduate Entry and foreign trained applicants)
  • Applicants who are nurses with a bachelor degree from another country need to contact Commission on Graduates of Foreign Nursing Schools at www.cgfns.org for analysis verifying BSN or equivalent. Please submit analysis to Admissions Office.
  • International applicants need official Test of English as a Foreign Language (TOEFL* or IELTS* Minimum required test scores http://gradschool.utoledo.edu/pages/testing.asp) score report (taken within two years), US citizenship or appropriate visa, and evidence of an earned bachelor degree as evaluated by one of the following services:

World Education Services, Inc.
International Educational Consultants
Educational Credential Evaluators, Inc.
International Education Research Foundation, Inc.
Spantran Educational Services, Inc.
