Secondary Application Process
After the preliminary screening of AMCAS applications, the Admissions Committee may request more information from students for further evaluation. Along with a set of forms, an application fee and letters of recommendations are required of the students. The secondary application process does not proceed until all the items are received.

UT Secondary Application Form (PDF format)
Prerequisite Completion Sheet (PDF format)

If you are a student that has been directed to the Web site after preliminary screening, please print, complete and return the above forms to the Admissions Office along with a non-refundable $80 application fee and a recent 2x2 photograph. Check and/or money orders should be made payable to "University of Toledo." AMCAS fee waivers will be honored.

Letters of recommendation must be on official university or college letterhead and from a Premedical Advisory Committee at the college or university from which you will graduate or have recently graduated. If no such committee exists, letters from three (3) faculty members with whom you have studied - at least two (2) in a science department is strongly recommended.

If you are not a recent college graduate, we will need in addition to the above, a letter of recommendation from someone who is familiar with your most recent activities since you left college. Additional letters of recommendation are accepted.

All secondary application material (including essays), prerequisite completion sheet and letters of recommendation should be sent to:

University of Toledo College of Medicine
Admissions Office
3045 Arlington Avenue, Mail Stop 1043
Toledo, OH 43614

Students will receive an email notification when their application is complete. The Committee will again review your application for an interview decision. Interviews are by invitation only and generally continue from October through March. If the interview is approved, you will receive an invitation to meet with members of our faculty. ALL APPLICATION MATERIAL MUST BE RECEIVED BY JANUARY 1, 2008 .
